SLSD Kindergarten Registration (2025-2026)

Miami Whitewater Elementary Families -

SLSD will open Kindergarten registration at 6:00am on February 3, 2025.  Keep in mind that to be eligible for Kindergarten in the 2025-2026 school year, children must turn five years old on or before August 1, 2025.  This is a change from previous years.  You will be able to find more information here: https://www.southwestschools.org/departments/enrollment/kindergarten-registration/   

PARENTS REGISTER USING ONLINE FORMS We will continue kindergarten registration for 2025-2026 on-line through the Final Forms portal.  All required forms must be completed, and all documents uploaded in Final Forms and will be time stamped electronically.   The registration will not be considered complete until all required (acceptable) documentation based on the Southwest Local School District Board Policy has been received. There will not be in-person appointments. All Forms must be completed online, no exceptions. Please Note-For the 2025-2026 School Year A Student must be 5 years of age on or before August 1st, 2025. 

Full Day Kindergarten - We will be offering Optional Full Day Kindergarten for 2025-2026 school year.  There will be a limited number of tuition-based spots available. The cost of tuition will be $3,960.00. (9 payments of $440.00) These registrations will be processed on a first come first served basis. Students will be placed into open spots based on the order in which registration is completed.

INSTRUCTION PACKETS - Packets include registration requirements and instructions for Final Forms online registration application. Packets are available on our website (www.southwestschools.org), click New Student Registration, and Kindergarten Registration (2025-2026) DOCUMENTATION.

Required documents - A copy of the birth certificate, proof of residency, photo id for parent/guardian and any court/custody documents including all pages, are all required for Enrollment. When you have completed the online registration and uploaded all documents in Final Forms, contact the District Registrar Lisa Williams at District Office (513) 728-8401 to make sure the documents have been received and are acceptable to complete your registration. These documents must be uploaded in Final Forms. There will be no in-person appointments. For proof of residency, parents will be required to provide ONE of the following: Mortgage Statement or Deed Property Tax Statement Lease Agreement That Isn’t Expired, (signed by both parties) A Month-to-Month Lease is not acceptable. A Current Water Bill in Parent/Guardian name only. No Electric, Phone or Cable Bills Accepted Notarized Southwest Local School District Affidavit of Residency along with a Water Bill in the same person’s name that signed the Affidavit of Residency. If the Affidavit of Residency hasn’t been fully completed it will not be accepted. The owner of the property must complete the Affidavit.
